A copy of your Idaho Driving Record can be obtained from any Idaho
DMV in-person. The Idaho Transportation Department requires the
you provide the following:
- A written request along with driver information and a $4.00 fee
(per record requested). The record can be certified for an additional

DRIVER'S LICENSE NUMBER, DATE OF BIRTH.
- The Idaho driver license number may be a nine-digit, alpha-numeric
number starting with two alpha characters followed by six numerical
characters and ending with one alpha character. Previous Idaho license
numbers began with 910, 920, or 940 assigned by the sheriffs' office,
or a social security number.
A driver license record and supporting document packet may also be
requested for a $9.00 fee. Requesters need to specify what documents
are needed. If you need a certified driver packet, the written request
must include the date of incident and a $17.00 fee.
You can also request a copy of your Idaho Driving Record via
mail. Simply visit the online Idaho DMV site and download a copy
of the Idaho Motor Vehicle Driver’s License Record Request form, fill
it out, print it, and mail it to:
